Coleman Provides Accessible Mental Health Care

Everyone who seeks mental health and addiction treatment should be able to access care without the barrier of cost. Coleman has worked to be in-network with many major insurance plans to make treatment affordable and accessible.

If you don’t have insurance, one of our financial counselors will explain the various assistance options available and will work with you to remove any financial barriers that may be preventing you from seeking the care you need.

Below is a partial list of the major insurance plans accepted at Coleman Health Services, depending on location.

Most health insurance plans provide mental health and substance use disorder benefits. However, the level of coverage offered will vary based on your specific type of health plan. The best way to find out coverage and understand what your cost may be and what services and providers your plan will pay for is to contact your insurance company directly by calling the customer service number on the back of your insurance card, reviewing your Summary of Benefits & Coverage document or by accessing your benefits through your insurance company’s website.

Our financial counselors are here to help you with all the financial components of your healthcare.

We understand the unknown cost of mental health and addiction services can be a deterrent for seeking treatment and we want to remove those financial barriers by offering financial assistance services to all of our clients; anyone who is uninsured or insured may be eligible for financial assistance.

Financial Counseling Services

We have the following financial counseling available:

Understand your insurance benefits and any out-of-pocket costs.

Learn about affordable payment options, including payment plans, discounts and request price estimates.

Answer questions about your bill.

Identify and apply for available state and federal assistance or other financial assistance programs offered through Coleman’s Financial Assistance Program.

Determine Medicaid eligibility and assist with Medicaid enrollment.

Learn if you are eligible for insurance through the Health Insurance Marketplace and help with the application process.

Understand Your Costs and Payment Options

A financial counselor can help review your coverage and benefits with you and inform you of any out-of-pocket costs like deductibles, co-insurance and co-pays. Additionally, a financial counselor can help determine whether your Coleman provider is “paneled” or commonly referred to as “In-Network” with your insurance plan.

Non-Covered Services

You will be responsible for paying the cost of any service deemed as not “medically necessary” or a non-covered service by your insurance plan. For example, Therapeutic Behavioral Health Services including Nurse and Case Management are not covered under Medicare and Commercial plans. Therefore, clients receiving these services are responsible for payment.

Self-Pay Rates

Coleman provides quality counseling and mental health services at a standard fee based on specific education, training and experience of our Coleman providers. Contact a financial counselor to discuss self-pay rates.

Price Estimates

A financial counselor can also provide you with a personalized price estimate based on your insurance and the services you are seeking. Contact a financial counselor to request a price estimate.

How to Apply For Financial Assistance

STEP 1
Download & Complete Application

For assistance completing the application, call or visit the financial counseling office at one of our Coleman Locations.

STEP 2
Provide All Required Documents

  • Verification of all household income for the past three months
  • Bank statements for the past three months
  • Copy of valid identification, driver’s license or social security card
  • Copy of birth certificate for each member of the household
  • Proof of residency

STEP 3
Return the completed application with copies of the required documents to our Financial Counseling Department.

Find Out If You Are Eligible for Medicaid

Coleman has partnered with Elevate Patient Financial Solutions to help clients without insurance determine if they are eligible for Medicaid and assist them with Medicaid enrollment. 

If you recently received services at Coleman and no insurance is on file, you may be contacted by an Elevate PFS Patient Advocate or be referred to MedData by one of our financial counselors.

To begin the application process, download and complete the Elevate PFS Intake Packet. * Please provide your preferred phone number and email address so that Elevate PFS can effectively assist you through the Medicaid application process.*

You will be asked to provide copies of some important documents that are required before Elevate PFS can submit your application to Medicaid, including: 

  • Valid identification, driver’s license or social security card
  • Proof of income for working adults in your household (paystubs, employer statement)
  • Copy of birth certificate for any minor children in your household
  • Proof of residency

Return your completed application to our Elevate PFS Patient Advocate or to the Financial Counseling Office at one of our Coleman locations. (need list of FC offices, contact info, hours)

Insurance Coverage Available Through the Health Insurance Marketplace

You may qualify for low-cost coverage through the Health Insurance Marketplace, including discounts on monthly premiums and out-of-pocket costs.

Apply for financial assistance, (add link to Financial Assistance Application). One of our Financial Counselors can help determine if you qualify and assist you with selecting the best plan for you and your family. 

Income eligibility requirements are based on Federal Poverty Level (FPL) and change each year. To find the latest FPL, visit ASPE.hhs.gov/poverty-guidelines.

Open enrollment in the Health Insurance Marketplace typically begins in November and ends in March.

Coleman’s Financial Assistance Program

Our financial assistance program is based on income level and other circumstances. If you don’t have health insurance, you must first apply for all applicable state/or federal aid programs before qualifying for one of Coleman’s financial assistance programs. 

If you qualify for a sliding fee scale discount, the discounted rate (percentage) you receive is applied to your total charges. Failure to make agreed-upon payments may result in disqualification from the program.
