Our Case Managers are credentialed behavioral and mental health professionals experienced in helping clients and their families in the recovery journey.

How Case Management Helps

Coleman is actively working to eliminate common barriers for those dealing with mental health issues. And our Case Managers work with you to receive the best services necessary for your recovery. They act as safety nets for individuals and families dealing with mental illness – dedicated to helping clients deal with life’s daily stressors. This may include a variety of things from illness education and coping skill teaching, securing safe housing, finding or keeping a job, enrolling in educational programs and keeping up to date with doctor visits, therapy and medication. That’s what makes Case Management so important. Coleman Case Managers work with a collaborative network of therapists, school staff, community partners and other mental health professionals and primary care providers – working toward a common goal to provide holistic,  individualized, integrated care. Case Managers also promote advocacy and outreach, and they help facilitate individual and family education and training.

Who Is Case Management For?

Our Case Managers work with all Coleman clients and families experiencing mental illness. Clients who benefit from Coleman Case Management services are those who experience severe and persistent mental illness who may also be struggling financially or with homelessness. Here they receive treatment in individual and small group sessions for ongoing assessment of needs, assistance in coping skills and managing issues, medication management and developing of daily living skills.

Case Management at Coleman Health Services

At Coleman, our Case Managers take a big-picture approach – this way, we are better able to prevent gaps in care and offer a targeted treatment plan designed to meet your specific needs. Our Case Managers take time to listen to each client’s unique concerns, in order to set personal recovery goals and develop a strategy for achieving them. Then, we work closely with each client to address their individual concerns and coordinate a client-focused plan of care with our integrated healthcare team.

Coleman Case Managers help clients schedule appointments, referrals and follow-ups. We support the transition from hospital to community care – helping reduce the need for emergency room visits. We also assist clients in practical matters that help support daily living, ongoing stability and recovery:

  • Arranging transportation
  • Creating a budget
  • Paying bills
  • Shopping for groceries
  • Cooking meals
