What is evaluation and diagnostic assessment?
This process consists of gathering information to assess an individual’s behavioral health needs. This helps clinicians determine the best service mix of mental health, addiction, and/or employment and vocational rehabilitation services to assist in recovery.
A diagnostic assessment is a clinical evaluation provided by a licensed professional in order to gather information to determine appropriate treatment based on the initial problem, current mental status and the diagnostic impression. The clinician will share the results of the diagnostic assessment with you in order to begin treatment planning.

How does Coleman evaluate and diagnose?
To receive a diagnostic assessment at Coleman, please bring photo identification and insurance card. Sliding fee options apply to those without insurance. Arrive a half-hour prior to your scheduled appointment in order to complete health questionnaire and financial intake.
